Post Hospital Care Journey, What to Expect
AFTER AN UNPLANNED EPISODE
You’ve found yourself in the hospital after an unexpected injury or illness. The hospital has advised that you’re not ready to go home and need Post Hospital Care for additional therapy and recovery. You or your loved one might be feeling emotionally shocked by this event and needing some guidance and support. We can ease your worry with customized recovery plans to get you home as quickly and successfully as possible.
AFTER A PLANNED SURGERY
Prior to a major procedure such as a joint replacement or heart surgery, you realize or are advised by your doctor that Post Hospital Care will be needed during your recovery period. As this is a pre-planned event, you have time to research your options to find the best fit for your needs and make necessary arrangements.
ONGOING CARE
You might find that after your therapy and recovery has ended, that you really appreciated the additional help and support. At this point you have the option to move to another level of care within the Marquis continuum; whether that be Marquis at Home, Assisted Living, Long Term Care, or Memory Care, all dependent on your health needs.

Manage Your Transition
Managing Medications
Many people have questions about how medications will be managed during their stay. You do not need to bring any medications or prescriptions with you. Your doctor will include all necessary prescriptions, including over the counter products, in your detailed medical orders. This process is managed by our in-house pharmacy. Our licensed nurses and nurse practitioners oversee and dispense all medications. When you are discharged, a nurse will review all your medications and determine those that will be transitioned back home with you.
PREPARING FOR A STAY
Having the personal items you want with you during your recovery can bring a sense of comfort and familiarity. Think of this like going to a furnished, extended stay hotel (with laundry service) and the things you might need or want there. You or your loved one likely has questions about how much a Post Hospital stay will cost and what will be covered by insurance. We’ve put together a list of some of the most frequently asked questions and their answers. However, the facility of your choice is always available to help walk you through the costs and process. Frequently asked questions
Admissions
You will be assigned an admissions liaison, whose job is to ensure the transition goes smoothly. The liaison will have access to your hospital records and will work with case managers to oversee and manage your transition from place to place. Once you arrive at the Marquis facility you will be warmly greeted by staff and shown to your room. Within the next 24 hours you can expect nursing, social service, dietary and therapy assessments as well as admission paperwork. It helps to be patient and prepared during this time, so bring insurance cards for admission paperwork. It’s also important to be mentally prepared for more therapy than the hospital. Remember that your ultimate goal through rehabilitation is to gain more independence and return to a lesser level of care.
Wills and Estates
Estate planning is an important arrangement for all aging people to have in place to avoid family disputes, legal fees and/or the government taking possession of assets. It’s also a chance for an aging person to make their wishes for their finances and health care known to their loved ones so everyone can be on the same page.
